NOS (New Old Stock) number plate from Trik Topz
There is no website on the packaging so I'm guess mid 1990's. That is also correct for the skull/8-ball/dice graphic.
Based on this being mid-school, I'm assuming it is designed for 4 piece bars (not Forklifter bars). This is further confirmed by the width of the stem cut out (2 5/8").
Overall dimensions: 14 1/8 at the top, 5 1/2" at the bottom, 7 1/8" tall
TOP PAIR OF MOUNTING HOLES (these are not visible b/c the stickers cover them up): Center of mounting hole to center of mounting hole: 8 1/8" (these are about 7/8" from the top and about 5 3/4" up from the stem cut out)
BOTTOM PAIR OF MOUNTING HOLES: Center of mounting hole to center of mounting hole: 8 1/8" (these are about 4 1/4" from the top and about 2 1/2" up from the stem cut out)
